In connection with her request that the Board waive a $120 and $720 late fee assessed for the late filing of the 10-G and 40-G report, Shelley Hebert states that she did not realize that a report was due on January 17, 2013 until she received her notice of delinquency letter from the Board. She asks that the Board forgive her for her error and lateness. She states that the error was truly unintentional. This is Ms. Hebert first campaign. She was an any other level candidate.

Each candidate for any other public office shall file reports of contributions in excess of two hundred dollars in the aggregate during the aggregating period.
