In connection with her request that the Board waive the assessed $400 late fee, Ms. Albritton states that she on February 5, 2013, she faxed in her 2012 Supplemental report and believed that the report had been properly submitted. She states that she was surprised when she received a Notice of Delinquency dated February 21, 2013 informing her that the Supplemental Report was late. She states that she contacted the Board's office and was told that staff would look for the report. She called the office again and spoke with Angela Newsome and was told that she would be assessed a late fee and she should explain the circumstances and request a waiver. Ms. Albritton states that she looked on our website, saw that the number listed on the website was different from the number where she faxed the original report and immediately faxed the report to the correct number. She concedes that she faxed the report to the wrong number (or an old number for the Board).A fax confirmation listing an incorrect number has been submitted as evidence of her attempt to file the report.
