RE:
Waiver request submitted by Jonathan Green, member of the White Castle Board of Aldermen, regarding a $1,500 late fee assessed for filing his 2009 Tier 3 candidate pfd days late. Untimley waiver request regarding a $1,500 late fee assessed for filing his 2009 Tier 3 annual pfd days late.
Facts:
Mr. Green states that his late filing was an honest mistake and an accidental oversight. He states that he is requesting a one-time waiver in connection with his failure to file his 2009 pfd.

The late fee order for the 2009 annual report was personally served on May 10, 2012. It took longer to serve him the late fee order for the 2009 candidate report, which was personally served on June 5, 2013.

Mr. Green never requested a waiver in connection with the 2009 annual pfd; however, it has been made a part of this request for the Board's information.
