Mr. Hidalgo did not file his November 2013 report timely due to his personal assistant having complications with her pregnancy. Documentation has been submitted supporting that she was in hospital for a prolonged period of time in December. Doctor's note indicates that she was not to return to work before January 14. Mr. Hidalgo requests that that a waiver be given because his assistant was the only person knowledgeable of the reports.
