Lois Lawrence is a former member of the New Orleans City Planning Commission who resigned effective February 19, 2014 due to health reasons and age. Ms. Lawrence is presently 80 years old and no longer has the energy nor endurance to maintain an active membership on the Planning Commission. Ms. Lawrence's health has deteriorated over the past few years and has had to cut back on her activities outside of the home. Ms. Lawrence is unable to drive anymore and relies on her children's assistance to get her to doctor's appointments and her dialysis treatments three times a week.
In 2013, Ms. Lawrence suffered a stroke and learned that she had sustained a number of mini-strokes over the past few years resulting in hypertension, diabetes, as well as sleep apnea and visual acuity. She is currently under 11 medications.
As a result, Ms. Lawrence has not been able to keep track of and act timely with regard to many things, none of this intentional. Ms. Lawrence has submitted some medical documentation, including medical bills. She had a friend help her compose this waiver request and if additional info is needed will request a relative to help her.

La. R.S. 42:1124.4 of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency. La. R.S. 42:1124.2.1 requires financial disclosure for each member and designee of a board or commission which has the authority to expend, disburse or invest $10,000 or more of funds in a fiscal year on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.

Suspend all but $250 based on future compliance. The $250 is payable within 30 days unless other payment arrangements are made. If other payment arrangements are not made and payment is not received within 30 days, the full amount of the late fee becomes due and owing.
