RE:
Waiver request submitted by Eugene Venzant, a member of the Bryceland Board of Aldermen, Bienville Parish, regarding a $1,500 late fee assessed for filing his amended 2012 Tier 3 annual pfd 172 days late.
Facts:
Mr. Venzant states that he has only an 8th grade education and did not understand the filing requirements. He originally filed his 2012 pfd on the wrong form and was assessed a $250 late fee, which the Board suspended all but $100, which he paid. He was then issued a notice of delinquency requesting he file on the correct form. Mr. Venzant has since resigned from the Board of Aldermen.
