Mr. Martin states that at the time the report was due, September 15, 2014, he was helping care for his grandchildren while his 16 yr old grandson underwent open heart surgery. Mr. Martin states that his grandson underwent open heart surgery the first week of school in August. His grandson was the oldest of four boys of his daughter, who is a single mother. The surgery took place in Dallas, TX. He states that his grandson was hospitalized for over a month with complications from an infection at which time he stayed in Dallas with his daughter to help. He states he forgot about everything else until his grandson was home safe.
Additional information was requested and a letter from Dr. Ashleigh Payne was received stating that his grandson was hospitalized at Medical City Children's Hospital from August 13, 2014 through August 20, 2014 and from September 3, 2014 through September 15, 2014.

La. R.S. 42:1119B(2)(a)(iii) requires that within thirty days after the beginning of each school year, any school board member or superintendent whose immediate family member is employed by the school board shall file a disclosure statement with the Board of Ethics stating the facts of such employment. Any person who fails to timely file a disclosure statement may be assessed a late fee of fifty dollars per day, not to exceed $1,500.
