In connection with his request that the Board waive the late fee assessed for the inaccurate filing of the report, Peter Galvan states:
- His report was filed timely;
- He is unable to determine any potential deficiency(ies) pursuant to the LFA Order in connection with R.S. 18:1495.5, as he (1) has been incarcerated since April 11, 2014, (2) has no internet/computer access, and (3) was not provided a copy of the report or statute for his review;
- His report was verified with his banking records and any reconciliation problem must have arose from a much earlier report, possibly 2011; and
- The CPA who prepared the report, Kim Kelly, was guided by the Ethics staff.

18:1505.4.A(1) Any candidate, the treasurer or chairman of a political committee, or any other person required to file any reports under this Chapter, who knowingly fails to file or who knowingly fails to timely file any such reports as are required by this Chapter may be assessed a civil penalty for each day until such report is filed. 18:1484.(2)(b). Each candidate for any other public office shall file reports of contributions in excess of two hundred dollars in the aggregate during the aggregating period.
