In connection with her request that the Board waive the late fee assessed for the late filing of the report, Carol Pettyjohn, report preparer for Jesse Fontenot, states that she had difficulty getting into the LEADERS program when she tried to file the 10-P report. She also stated that she was not aware that she was required to file a 10-G report because when the report was due on November 26, 2014, Mr. Fontenot was not a candidate.
