RE:
Waiver request submitted by Kristen Sienna Carter "Krissy" Jolly, candidate for Clarks Board of Aldermen - Caldwell Parish in the November 4, 2014 election, regarding a $1,500 late fee assessed for filing her amended 2013 Tier 3 Candidate PFD 82 days late.
Facts:
Ms. Jolly states that she does not recall receiving the Notice. She explained that she is unable to go to the post office during business hours because of her work schedule. Therefore, her mother picked up and signed for the certified mail. Ms. Jolly further states that the imposition of a late fee would create a financial burden. A request has been sent for supporting documentation.

Her father submitted a letter stating that Ms. Jolly and her three children have been living with him and his wife full-time out of necessity due to financial reasons for approximately 3 years.

Ms. Jolly received the notice of delinquency to amend requesting her to disclose income received from a political subdivision by exact dollar figure.
