Waiver request submitted by Michael F. Abendroth, former member of the Madison Parish Hospital Service District, regarding a $600 late fee assessed for filing his 2014 Tier 2.1 Annual PFD 12 days late. |
|
Mr. Abendroth states that he was unaware of the situation because he was out of town at the time the Notice was issued. A review of the green card indicates that Liz Miller signed for the Notice of Delinquency. He states that his service ended in January 2015.
