RE:
Waiver request submitted by Deborah Mabile "Debbie" Settoon, former member of the Southeast Regional Airport Authority, regarding a $650 late fee assessed for filing her 2013 Tier 2.1 Annual PFD 13 days late.
Facts:
Ms. Settoon states she was involved in a car accident around June 2015 and she has been receiving medical treatment and physical therapy. However, the note provided to us from her doctor, dated June 17, 2015, states she may return to work immediately and no excessive stair climbing.

Ms. Settoon's service on the board ended in 2013. Therefore, the 2013 pfd was her final report.
