In connection with her request that the Board waive the late fee assessed for the late filing of the report, Darleen Nicole Sheppard states that it was her true intent to follow all guidelines set out by the Ethics department. She states that her mother, who was her report preparer, found a lump in her breast on November 6, 2013 and they immediately began searching for facilities that could examine her and perform necessary testing. She did not complete mandatory filing timely because of this, however, as soon as notification was received that the report was past due, she immediately submitted the report.
Ms. Sheppard states that she was her mother's primary caregiver during her chemotherapy and radiation treatments before she passed away. Their lives were taken over by this horrible disease and mistakes were inadvertently made because of it.

18:1505.4.A(1) Any candidate, the treasurer or chairman of a political committee, or any other person required to file any reports under this Chapter, who knowingly fails to file or who knowingly fails to timely file any such reports as are required by this Chapter may be assessed a civil penalty for each day until such report is filed. 18:1484.(2)(b). Each candidate for any other public office shall file reports of contributions in excess of two hundred dollars in the aggregate during the aggregating period.
