Mr. Applekamp states that he tries to make sure his reports are filed each month by the deadline; however, his mother passed away on September 23, 2015. She was in the hospital for approximately a month, and he had been in Charleston, SC, with her for about a week before her passing. He returned to Oklahoma, and to work, on Monday, September 28, 2015. Upon reviewing his task list, he realized he missed filing his report.
