RE:
Waiver request submitted by Lloyd Henry Benson, II, Recreation and Parks Commission - East Baton Rouge Parish, regarding a $500 late fee assessed for filing his amended 2014 Tier 2.1 annual PFD 10 days late.
Facts:
Mr. Benson states that he was unaware of the requirement to disclose his wife's income from the Celerity Dalton Elementary Charter School. He further states that now that he has a better understanding of what is required, he will file all future disclosure statements correctly.

It should be noted that he filed his 2015 pfd late and he also failed to properly disclose his wife's income after being advised such was required on the pfd. A NOD-amend has been issued.
