RE:
Waiver request submitted by Wilmer Joseph "Wil" Jacobs, Jr., a former member of the Audubon Commission, regarding a $1,500 late fee assessed for filing his 2014 Tier 2.1 annual PFD 237 days late.
Facts:
Mr. Jacobs states that he is involved in many public and community services and takes those obligations quite seriously. He states that he was not attempting to deceive the public and states that it was simply and oversight. He also states that he has timely filed all of his previous disclosure statements and requests consideration when reviewing his waiver request.

Mr. Jacobs also states that the imposition of a late fee would cause financial hardship. Correspondence was sent to Mr. Jacobs requesting that he submit supporting documentation.
