RE:
Waiver request submitted by Timmy Thomas Hymel, Morgan City - City Council (St. Mary Parish), regarding a $1,500 late fee assessed for filing his amended 2015 Tier 3 annual PFD 34 days late.
Facts:
Mr. Hymel states that he did include Schedule B in his initial filing of his PFD (covering 2015). He explains that the schedule apparently did not go through with the other pages in his fax transmittal. Mr. Hymel states that he filed a second amendment thinking that all income (received from the State, political subdivisions of the State, and gaming interests) was disclosed; however, he later learned that he omitted his income from Morgan City. Mr. Hymel states that he did not intentionally omit the information and once made aware of the omission, filed the correct amendment.
