RE:
Waiver request submitted by James Cormier, member of the Lafayette Parish Communications Commission, regarding a $1,500 late fee assessed for filing his amended 2015 Tier 2.1 annual PFD 99 days late.
Facts:
Mr. Cormier stated that he serves on the commission because of his employment with the Office of State Police. He stated that he was not aware of having to disclose his employment income (with the State Police). He thought it pertained only to income that he would have received from his service on the commission. He takes full responsibility for the omission and states that it was in complete error- not malice or ill intent.
