RE:
Waiver request submitted by Jerry Bowman Jr., Shreveport City Council, District G- Caddo Parish, regarding a $2500 late fee assessed for filing his amended 2015 Tier 2 annual pfd 34 days late.
Facts:
Mr.Bowman stated that he did not personally sign for the NOD. His wife signed it for him and she didn't immediately give it to him because she thought it was issued in error. The NOD requested he disclose his ownership interest in Five Star Cleaning.

Mr. Bowman explained that he received a Notice to Rescind a late fee on December 4, 2015 and believed that it pertained to the late fee in question. A NOD (and subsequent late Fee) was issued for Bowman failing to disclose income from a political subdivision. It was later learned that no income was received during 2014 and the late fee was rescinded.
