RE:
Waiver request submitted by Monica Alejandra Candal Rahim, McDonough City Park Academy, regarding a $1500 late fee assessed for filing her 2015 Tier 3 annual pfd 40 days late.
Facts:
Ms. Rahim stated that she uses her parent's mailing address to receive mail because she and her husband (a doctor who is reassigned frequently) sold their house and moved to California but needed a consistent address to assure that they receive all of their mail. She explained that the notice of delinquency was signed for by her mother. She opened the notice when she traveled to her parent's home for the holidays. She also stated that she attempted to file the disclosure statement on January 7, 2017; however, it was not until January 30, 2017 that she was successful in transmitting the disclosure statement.

Ms. Rahim requested that the late fee be waived. She also requested that she be contacted via email because she and her husband will likely be moving several times in the near future.
