Wysinger Cleveland was issued a NOD requesting that he amend his 2016 Tier 3 annual PFD to disclose information regarding his wife, including her employment information and income as well as income he received from the Town of Richwood and University Health Hospital. Mr. Cleveland responded that he and his wife have been separated since November 2014, but have chosen not to divorce for personal reasons. As such, he states he is unable to provide his wife's employment or income information.
He has also not provided information that pertains to his income.

La. R.S. 42:1124.3C(1)(a) requires the name, address, type and amount of each source of income received by the individual or spouse, or by any business in which the individual or spouse, either individually or collectively, owns an interest which exceeds ten percent of that business, which is received from the state or any political subdivision or for services performed for or in connection with a gaming interest.

Advise Wysinger Cleveland that he must disclose all information required pertaining to both himself and his wife regardless of the separation.
