RE:
Waiver request submitted by Velma Hendrix, a member of the Melville Town Council, St. Landry Parish (Pop. 1,041), regarding a $1,500 late fee assessed for filing her 2015 Tier 3 Annual PFD 98 days late.
Facts:
Ms. Hendrix states that she was not aware of the requirement to file PFDs every year. She has filed 3 other PFDS with this office. She also states that the amount of the fine exceeds her compensation for service on the board. She states that the imposition of the late fee would create a financial hardship because she has extremely high medical expenses.
