RE:
Waiver request submitted by Yvette Young Semmes, Upper Audubon Security District, regarding a $1500 late fee assessed for filing her 2015 Tier 2.1 annual pfd 180 days late.
Facts:
Ms. Semmes stated that she takes the requirement to file a PFD very seriously. She explained that she volunteered to serve on the District to help make her community a safer place. She explained that during calendar year 2016 she lost her mother-in-law and that contributed to her not taking care of the filing.

Correspondence was sent to Ms. Semmes requesting copies of the obituary notices.
