RE:
Waiver request submitted by Bonnie M. Crockett, with the Columbia Town Council - Caldwell Parish (Pop. 390), regarding a $1500 late fee assessed for filing her 2016 Tier 3 annual PFD 114 days late.
Facts:
Ms. Crockett stated that she assumed her attempt to electronically file was successful; however, if this agency does not have the PFD, the attempt must have failed. Ms. Crockett also stated that around the time the NOD was received her granddaughter (who lived with her) was killed in an automobile accident (September 2017). In December 2017, Ms. Crockett's brother died.

Ms. Crockett request that consideration be given when her waiver request is reviewed.

The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
