RE:
Waiver request submitted by Susan T. Herring, former member of the Homer Board of Aldermen - Caliborne Parish (Pop. 3,237), regarding $1500 late fee assessed for failing to file her 2016 Tier 3 annual PFD.
Facts:
Ms. Herring served for 6 months in 2017. She was unaware of the requirement to file this form. Upon receiving notice from our office, it took her some time to figure out how to complete and submit the statement online. She states she submitted a statement online, on April 10, 2018 and that the form was successfully uploaded. She is asking for a waiver, as she was not elected nor did she seek a full-term to the office. She states she was truly unaware of the requirement to file this form.
