RE:
Waiver request submitted by Adam Daniel James Harris, with the Springhill Board of Alderman, District 4, Webster Parish (Pop. 5,269), regarding $1250 late fee assessed for filing his 2015 Tier 3 annual PFD 25 days late and a $1500 late fee assessed for filing his 2016 Tier 3 annual PFD 202 days late.
Facts:
Mr. Harris was unclear that anything had to be filed. According to Mr. Harris, once he confirmed he needed to file, the late fee had already been assessed. He states the process was confusing. He had to be walked through the process and once that occurred he was able to file the Tier 3 paperwork.
