RE:
Waiver request submitted by Mathew Rickey Hardy, former member of the Board of Pardons and former State Representative, current member of the Lafayette North Redevelopment Authority, regarding a $2,500 late fee assessed for filing his amended 2016 Tier 2 annual PFD 121 days late.
Facts:
Mr. Hardy states that his failure to file was not planned or intended, but was simply an unintended oversight on his part. He states that he is no longer in public office and that his position with the Lafayette Parish Sheriff's office precludes him from running for any office. He states that he has no previous late fines and has filed his paperwork when required.
