Waiver request submitted by Jayme Lee Polotzola Johnson, with the Melville Town Council, St. Landry Parish (Pop. 1041), regarding a $1500 late fee assessed for filing her 2018 Tier 3 annual pfd 61 days late. |
|
Jayme Johnson is seeking a waiver request for the following reasons:
Ms. Johnson stated she was elected as a council member for the town of Melville effective January 2019, but was unaware she was required to submit a 2018 PFD or annually file PFD's until she spoke with a staff member who explained the process. Ms. Johnson stated now that she understands the procedures, she "promises" to "follow rules and always complete things in a timely manner". She asked that the Ethics Board "please waive this $1500 penalty since she was truly unaware she needed to file." Once Ms. Johnson received her late fee notice she filed a compliant PFD the same day.
