RE:
Waiver request submitted by Eric D. Tarver, with the Calcasieu Parish School Board, District 8, regarding a $2500 late fee assessed for filing his 2018 Tier 2 annual pfd 159 days late.
Facts:
Eric Tarver is requesting a waiver for the following reasons:

After contacting the BOE and speaking with the PFD Director, Mr. Tarver learned that although he faxed his 2018 PFD a couple of times, the faxes weren't completed. Mr. Tarver stated that he understands that it his responsibility to ensure that the Board of Ethics receives his disclosure statement. However, considering that he did attempt to submit the pfd on time, and that he does not have a history of filing late; he is seeking any leniency that can be granted regarding the $2500 late fee. He also stated he would be very grateful if the fine could be waived. This is Mr. Tarver's first late fee.
