Floydean White is requesting a waiver for the late filing of her 2017 Annual PFD. Mrs. White states she overlooked the Late Fee Assessment she received. She states she did not intend to violate any Ethics Rules as an elected official. Her husband is retired and has high blood pressure; and in 2018 he began showing signs of dementia, that rapidly worsened. Both conditions have caused an extra burden on her trying to keep him safe and properly nourished.Caring for him has taken up more of her time, creating less time to attend business affairs. She finally was able to get Home Health Care for him, which has helped; allotting her time to focus again on their business affairs. Mrs. White stated at times they do not have enough money to pay bills or medical expenses since they only receive Social Security; and often utilities are turned off. Mrs. White asked the board to please consider waiving the late fee, as she will prove to the Board that she will never be delinquent again.
Mayor Terry Hoof is writing a Waiver Request Consideration Letter on behalf of Council member, Mrs. Floydean White. Mayor Hoof stated Mrs. White is a dedicated council member and has helped the town grow economically, environmentally and educationally. Mayor Hoof acknowledged Mrs. White's husband's medical condition over the past two years, and stated with his condition and their lack of income/employment, her late fee would be an additional financial burden for them. Mayor Hoof stated he has financially assisted the Whites in the past and that Mrs. White would not avoid filing responsibilities on purpose.

La. R.S. 42:1124.3
requires persons holding a public office who represent a voting district having a population of less than 5,000 and each member of the governing authority or management board of a charter school created pursuant to Chapter 42 of Title 17 of the Louisiana Revised Statutes of 1950 to file a financial disclosure statement on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
