Carroll Daniels was required to file a school board disclosure statement with 30 days of the first day of school for the 2010-2011 school year. The disclosure statement was not filed until September 2011. However, a notice of delinquency was not issued to Mr. Daniels within four (4) days of the report due date. As such, Mr. Daniels was only assessed a $200 late, representing the four days late. Further, once Mr. Daniels did receive a notice of delinquency, he promptly filed the required report.

La. R.S. 42:1119B(2)(a)(iii) requires that within thirty days after the beginning of each school year, any school board member or superintendent whose immediate family member is employed by the school board shall file a disclosure statement with the Board of Ethics stating the facts of such employment. Any person who fails to timely file a disclosure statement may be assessed a late fee of fifty dollars per day, not to exceed $1,500.
