In connection with his request for a waiver of the $480 late fee assessed for the late filing of a 10-P campaign finance report, Gillis R. Windham, states that sometime in July 2011. At that time, he believed that he had downloaded the LEADERS software from the website and obtained an id and password. He states that on October 12, 2011 he tried to submit his report and realized that had not obtained his password and id. He maintains that on October 13

he was very ill with a stomach virus and could not attend work. Therefore, he asked a co-worker to file the report on his behalf. An affidavit was received on October 17, 2011 and he was able to obtain an id and password on October 20, 2011. The report was immediately filed once he was in receipt of an id and password. Mr. Windham is a district level candidate. This is his first campaign. He was an unsuccessful candidate.

18:1505.4.A(1) Any candidate, the treasurer or chairman of a political committee, or any other person required to file any reports under this Chapter, who knowingly fails to file or who knowingly fails to timely file any such reports as are required by this Chapter may be assessed a civil penalty as provided in R.S.42:1157 for each day until such report is filed.
