In connection with his request for a waiver of the $200 and $320 late fees assessed for the late filings of a Special and 10-G campaign finance reports, John R. Pizzolatto states that he was informed by a person from the Campaign Finance division that he used the wrong report for his Special report. He further states that he made a simple error by not timely reporting a contribution. Mr. Pizzolatto is a district level candidate. This is his 6

campaign. He was an unsuccessful candidate.
