In connection with his request for a waiver of the $420, $480, and $1,680 late fees assessed for the late filings of a Special, EDE-P, and 10-G campaign finance reports, Otis Chisley, Jr. states that on or about October 17, 2011, he became ill. On October 24, 2011, he was admitted into the hospital, had major surgery, and was hospitalized for five days. He maintains that he was unable to reach a friend who he had assist him with the filings of his reports. He was under doctor's care, on pain medication, and off from work for several weeks. When he returned to work, he received the notices from the Campaign Finance division. The reports were filed as soon as possible. Mr. Chisley is a district level candidate. This is his third campaign. He was an unsuccessful candidate.
