In connection with his request for a reconsideration of waiver of the $780 late fee assessed for the late filing of a 30-P campaign finance report, David Rogoz maintains that he timely submitted his report. In addition, Mr. Rogoz has provided a fax log from Midtown Auto Title, the location from which he claims his 30-P report was faxed. A research of the fax number by the Ethics IT division revealed that a 30-P report was not received on September 21, 2011 from the number shown on the fax log provided by Mr. Rogoz. Staff placed a call to Mr. Rogoz on March 22, 2012, to find out what fax number he used to fax his 30-P report. Mr. Rogoz was unavailable and a message was left. Mr. Rogoz is a district level candidate. This is his second election. He was an unsuccessful candidate.

18:1505.4.A(1) Any candidate, the treasurer or chairman of a political committee, or any other person required to file any reports under this Chapter, who knowingly fails to file or who knowingly fails to timely file any such reports as are required by this Chapter may be assessed a civil penalty as provided in R.S.42:1157 for each day until such report is filed.
