In connection with her request for a waiver of the $480 late fee assessed for the late filing of an EDE-P campaign finance report, Zaine "The Queen" Kasem, states that she called a few days before her report was due and was told that she did not have to file until November 9, 2011. She then filed her report on the day she believed it was due, during which time she was told that her report was timely as it was being stamped. She further states that this is her first late fee assessment and ask that the Board either waive or reduce the late fee. Ms. Kasem is a district level candidate. This is her second campaign. She was an unsuccessful candidate.
In addition, this matter was deferred from the April 20, 2012 meeting, to allow the candidate to provide medical documentation supporting her claim that on March 24, 2012, she was attacked by a cow and sustained injuries. Correspondence was received by staff from Ms. Kasem via email on Friday, April 20, 2012. A copy of the email is attached for reference. Staff has requested medical documentation from Ms. Kasem and will be presented to the Board at the meeting.

18:1505.4.A(1) Any candidate, the treasurer or chairman of a political committee, or any other person required to file any reports under this Chapter, who knowingly fails to file or who knowingly fails to timely file any such reports as are required by this Chapter may be assessed a civil penalty as provided in R.S.42:1157 for each day until such report is filed.
