Agency Tracking No. 2023-417 <br /> Page 4 of 8 <br /> 9.0 <br /> At the January 7, 2021 District's Board Special Meeting, Mary Trichell participated in <br /> adopting a Resolution amending the Tune 23, 2020 Hospital Bond Resolution. This Resolution <br /> redefined definitions on the original Resolution. No Disclosure Recusal Form was filed with the <br /> BOE pursuant to La. R.S. 3 9-W 123 3.1. <br /> 106 <br /> On December 30. 2020, as Chairman for the District, Mary Trichel l executed the Closing <br /> Certificate and Receipt for the `Bond Anticipation Dote, Series 2020, of the Issuer, dated the date <br /> hereof, issued in the form of a single fully registered bete, numbered R-1, in the denomination of <br /> $271497 (64$2454421,000 of Hospital Revenue Bonds,Series 2022A,and$3,055,000 of Hospital <br /> Revenue Bonds (Taxable), Series 2022B"). Delta Bank was the Dote and Series B Bond purchaser <br /> of this 7,497,000 loan. No Disclosure Recusal Form was Filed with the BOIL pursuant to La.R,S. <br /> 39:1233.1. <br /> 11. <br /> Mary Tricbell failed to file annual financial disclosure statements pursuant to La.. R.S. <br /> 42:1114 covering calendar years 2019, 2020, and 2021. <br /> Iv. <br /> PROVISIONS of AW: <br /> The following provisions of law are implicated by the alleged wets stated herein: <br /> (a)La.R.S.42:11 t I 2)No public servant and no legal entity in which the <br /> public servant exercises control or owns an interest in excess of twenty-five percent, <br /> shall receive anything of economic value for or in consideration of services rendered, <br /> or to be rendered.,to or for any person during his public service unless such services <br />
