R.S. 3 9-W 123 3.1. <br /> 106 <br /> On December 30. 2020, as Chairman for the District, Mary Trichel l executed the Closing <br /> Certificate and Receipt for the `Bond Anticipation Dote, Series 2020, of the Issuer, dated the date <br /> hereof, issued in the form of a single fully registered bete, numbered R-1, in the denomination of <br /> $271497 (64$2454421,000 of Hospital Revenue Bonds,Series 2022A,and$3,055,000 of Hospital <br /> Revenue Bonds (Taxable), Series 2022B"). Delta Bank was the Dote and Series B Bond purchaser <br /> of this 7,497,000 loan. No Disclosure Recusal Form was Filed with the BOIL pursuant to La.R,S. <br /> 39:1233.1. <br /> 11. <br /> Mary Tricbell failed to file annual financial disclosure statements pursuant to La..
