R.S. <br /> 42-11112B(3) by participating in three (3) transactions involving Madison Parish Hospital Service <br /> District in which Delta Bank had substantial economic interest while Mary Trichell was employed <br /> with Delta Bank. <br /> 0 <br /> Based on the foregoing alleged facts herein, Mary Trichell, while serving as a member of <br /> the Board of Commissioners of the Madison parish Hospital Service District, violated La.. R.S. <br /> 42:1114 by failing to file annual financial disclosure statements for calendar years 2019,2020, and <br /> 2021. <br /> WHERE4 FORE, the Louisiana Board of Ethics respectfully prays that <br /> (a) The Ethics Adjudicatory Burd conduct a hearing on the foregoing charges; <br /> (b) The Ethics Adjudicalory Board determine that Mary Trichell violated La. R.S. <br /> 42:1.111 ( d , La.R.S. 42:1112B(3), and/or La.R.S. 42;1114, with respect to the <br /> forgoing alleged facts; <br /> (e) The Ethics A judicatory Board assess appropriate penalties in accordance With the <br /> Louisiana Code of Govemmental Ethics; and, <br /> (d) The Ethics Adjudicatory Board only send electronic notices in this natter to the Lead <br /> Trial Attorney as de s i gnated above. <br />
