11. <br /> La. R.S. 18:1505.1C provides that the failure to disclose or the failure to accurately <br /> disclose any information required to be reported by the Campaign Finance Disclosure Act shall <br /> constitute a violation of the Campaign Finance Disclosure Act. <br /> 12. <br /> Based on the foregoing facts, Jerome Richard failed to disclose or accurately disclose <br /> contributions accepted and expenditures made totaling approximately $37,000 related to his <br /> personal use of his campaign funds on campaign fmance disclosure reports in violation of La. <br /> R.S. 18:1505.1C. <br /> 13. <br /> As evidenced by his signature below, Jerome Richard knowingly, and of his own free <br /> will (a) agrees that the violations are factually supported by stated facts herein; (b) agrees that he <br /> violated La. R.S. 18:1505.2I(1) and La. R.S. 18:1505.1C; (c) acknowledges that upon his <br /> signature being affixed to this consent decree, this document will be available to the public as a <br /> public record; (d) agrees that this order and decree is fmal, waives his right of appeal or to <br /> otherwise seek a return of the payment as the penalty; (e) agrees to pay the penalty assessed <br /> herein; (f) agrees to remove himself as a signatory on his current or future campaign accounts; <br /> (g) agrees to amend his campaign fmance disclosure reports to accurately disclose all <br /> contributions received and expenditures made no later than December 31, 2017; and otherwise to <br /> comply with any other term or condition in the following order and decree of the Board. <br /> Page 3 of 6 <br />
