Ethics Board Docket No. 2020-499 <br /> Page 2 of 8 <br /> 2. <br /> The BOE is authorized to offer a Consent Opinion to Edward Calloway by virtue of Section <br /> 1141C(3)(f)of the Ethics Code. <br /> III. FINDINGS OF FACT: <br /> 3. <br /> G.B. Cooley Hospital Service District ("Hospital"), 211 North 3`d Street, Monroe, <br /> Louisiana, is a political subdivision of the state created in 1962 by Ouachita Parish Police Jury <br /> Ordinance No. 7127 pursuant to La.R.S. 46:1051 and La. R.S. 46:1067. <br /> 4. <br /> On September 11, 2009, the Hospital hired Edward Calloway as a full time IT technician <br /> in the Finance Department. <br /> 5. <br /> On November 8, 2016, the Hospital promoted Mr. Calloway to Chief Financial Officer,a <br /> position he held until the Hospital terminated his employment on October 23, 2019. <br /> 6. <br /> As the Hospital's Chief Financial Officer, Mr. Calloway's duties and responsibilities <br /> included developing and implementing procedures for financial operations;providing supervision <br /> and oversight of accounts receivable, accounts payable, payroll, IRS 1099 reports, depreciation <br /> schedules, the general ledger, budgets, financial statements and cost reports; information <br /> technology support and communicating with the Hospital's Chief Executive Officer regarding <br /> financial operations. <br />
