LOUISIANA BOARD OF ETHICS <br /> CONSENT OPINION <br /> RE: IN THE MATTER <br /> OF JACOLBY OCTAVE BOARD NO: 2019-852 <br /> The Board of Ethics (the "BOE"), pursuant to the authority contained in the Louisiana <br /> Code of Governmental Ethics (hereinafter referred to as the`Ethics Code"), La. R.S. 42:1101, et. <br /> seq., and particularly Section 1141 of the Ethics Code, conducted a confidential investigation <br /> concerning information that Jacolby Octave, while serving as a Board Member for the Vacherie <br /> Volunteer Fire Department No. 1, violated La. R.S. 42:1111A(1)(a) when he received payments <br /> from the Department that he was not duly entitled to receive for the performance of his job duties <br /> and responsibilities as a Board Member. Additionally, Jacolby Octave, while serving as a Board <br /> Member for the Vacherie Volunteer Fire Department No. 1, violated La. R.S. 42:1112(A), when <br /> he signed checks issued to himself, when he knew he had a substantial economic interest in the <br /> transactions involving the Vacherie Volunteer Fire Department No 1. <br /> On the basis of information obtained during the investigation,and with the concurrence of <br /> Jacolby Octave,the BOE and Jacolby Octave enter into this Consent Opinion as follows: <br /> I. <br /> PARTIES: <br /> The Parties hereto are: <br /> Jacolby Octave, a person of the fall age of majority, domiciled in the Parish of St. <br /> James, with a last known address of 21119 Highway 20, Vacherie, Louisiana <br /> 70090. <br />
