RE:
Request to reconsider the Board's denial of a waiver request submitted by Ms. Donnica Conway, former member of the Louisiana Developmental Disabilities Council, regarding a $1500 late fee assessed for filing her amended 2016 Tier 2.1 annual pfd 195 days late.
Facts:
At the February 2019 meeting, the Board considered a waiver for two late fees. The Board suspended all of the $1500 late fee associated with the 2015 Tier 2.1 report and declined to waive the $1500 late fee associated with the 2016 Tier 2.1 report and offered a payment plan.

Ms. Conway now states that the $1500 late fee will create a financial hardship. She has submitted supporting documentation.

Initial Information

Ms. Conway states that she faxed her PFD on the due date; however, it was unreadable. She was not aware of the problem but corrected it as soon as she was notified. She also states that she is a recent college graduate and a first year teacher and does not have the income to pay the late fee.
