Ryan Verret is requesting a waiver for the late filing of his final Tier 2.1 Annual PFD for the following reasons:
Mr. Verret stated as a new member of a volunteer board organization, he was not fully aware of the final filing requirements. Had he been aware, he certainly would have completed the PFD. Mr. Verret stated he has no recollection of receiving the NOD requesting he provide the 2018 PFD Statement. He has quite a busy household with a large family. He did not see or open a letter that was given to him with instructions to file.
He was caught off guard by the timeline and penalties attached to not filing the disclosure for his volunteer service. His brief service on that board was interrupted many times, not for his lack of wanting to serve his community but, primarily with his inability to be present for personal (family) circumstances. He took on a role he could not fulfill. His responsibilities of having two jobs and preparing for his fifth child were overwhelming. He understands the role and responsibilities of the Board of Ethics to ensure public confidence in our service organizations. However, he would greatly appreciate (as well as his family) if his penalty of $1500 was waived. Mr. Verret thanks the Ethics Board for their service to the State of Louisiana and for considering such a personal request at such a burdensome time for so many struggling to support their families and current responsibilities.

La. R.S. 42:1124.2.1
requires financial disclosure for each member and designee of a board or commission which has the authority to expend, disburse or invest $10,000 or more of funds in a fiscal year on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
