Stephen Weeks is employed with Institutional Investment Management for William Blair Building Company. Mr. Weeks filed a Retirement System Disclosure statement disclosing expenditures made on retirement officials. After filing the disclosure untimely, a Late Fee Order was issued. Thereafter, Mr. Weeks submitted a waiver request stating the disclosure was filed in error. However, no explanation was ever given as to what error lead to the filing, i.e., whether the expenditures were not actually made or the date of the expenditures initially given was in error.

La. R.S. 42:1114.2 A. Each person who has or is seeking to obtain contractual or other business or financial relationships with a state or statewide public retirement system shall file with the Board of Ethics, in the manner provided in this Section, a report of all expenditures for a retirement official or retirement officials.
