Patrick H. Martin, V, stated this was his first time filing a disclosure and he simply overlooked Schedule B, since his public employment was already disclosed on the Schedule A page. It was not his intent to avoid disclosing his income information. Mr. Martin stated when he received the NOD, he was working from home due to the Covid-19 Pandemic and did not have a printer or a scanner. Once he completed the amendment, he unfortunately placed the form to the side and forgot to submit it. He requests the waiver as he only failed to disclose his actual salary level from his public employment with LSU, which is available from LSU as a matter of public record. He stated he has been a public employee for most of his career, serving as an Assistant District Attorney, General Counsel for the Legislative Auditor, an attorney for the Governor, and in several capacities for LSU. He takes compliance with the Ethics Code very seriously, and is embarrassed by the fact that he (unintentionally) failed to fully comply with the Code of Ethics. Mr. Martin stated "while he makes a good living from LSU at the moment, a $1,500 late fee would be a hardship on his family." He can assure the BOE that the sheer embarrassment at finding himself in this situation will absolutely deter him from making such a mistake in the future.

La. R.S. 42:1124.3
requires persons holding a public office who represent a voting district having a population of less than 5,000 and each member of the governing authority or management board of a charter school created pursuant to Chapter 42 of Title 17 of the Louisiana Revised Statutes of 1950 to file a financial disclosure statement on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.

Suspend all but $800 based on future compliance with the Code of Ethics. The $800 is payable within 20 days and if payment is not received within 20 days, the full amount of the late fee becomes due and owing.
