The report was completed and mailed to a physical address listed on one of the forms on Monday, November 9, 2020, along with required processing fees in the amount of $100.00. It was returned by the agency listed on one of the forms stating as incorrect and provided them with the correct address. They resubmitted the documents after the notification of the missing documents on 12/15/20 and has since complied with all ongoing requests and obligations prior to the timeline.
