Blaise W. Smith requests a waiver and states the following:
The Late Fee of $780.00 should be waived due to his not receiving notice until 19 months later. He received the new notice July 16, 2021, 14 days after being notified of this issue in a phone conversation with staff.
He states he filed the 2020 Annual report online electronically on 2/17/21. (NOTE: The 2020 report was NOT filed on 2/17/21, the report he was referring to was his 2019 Supplemental filed on 2/17/20). When he was made aware of the problem he contacted staff to correct any problems at that time. He had to get IT to assist him because the online filing was not going through the system at that time.
