Charles Deliberto requests a waiver and states the following:
On January 11, 2021, when preparing to file his 40-G report he fell ill which was later diagnosed as Covid 19. He was attempting to track down several missing addresses to ensure that the filing was complete when his symptoms began. The acute phase of the illness was particularly difficult for him and unfortunately the 1/14 deadline passed during this time. After recovering from Covid, he erroneously believed he already filed the report in a timely manner. He states that he has a well documented post viral illness which includes problems with memory, cognition and many other neurological problems.
