Waiver request submitted by Ernest Glynn "Ernie" Roden, North Hodge Board of Aldermen (Pop. 296) / Jackson Parish, regarding a $500 late fee assessed for filing his amended 2019 Tier 3 annual pfd 32 days late.
Facts:
Ernest G. Roden is requesting a waiver for the following reasons:

Mr. Roden stated he suffered severely after contracting the Coronavirus in May 2021. Both he and his wife were hospitalized from 5/14/2021 until 5/24/2021, at the Jackson Paris Hospital Covid-19's IC Unit. During his hospitalization, Mr. Roden "coded" (went into cardiac arrest or was not breathing). After being discharged, he was admitted to the Forest Haven Nursing Home and Rehabilitation Center (24 -Hour Nursing Care Services) from 5/24/2021-7/15/2021. During his rehabilitation stay, Mr. Roden, "coded" 2 more times, which resulted in his treatment being changed to total assistance care. As his rehab progressed his treatment was gradually modified to independent care for the remainder of his rehab stay. On 7/15/2021, Mr. Roden was released and referred to the Lagniappe Home Health Agency for in-home care services, provided by NP, Kim Brunson until October 2021. Mr. Roden completed his amendment as soon as he was physically able to do so.

Law:
La. R.S. 42:1124.3 requires persons holding a public office who represent a voting district having a population of less than 5,000 and each member of the governing authority or management board of a charter school created pursuant to Chapter 42 of Title 17 of the Louisiana Revised Statutes of 1950 to file a financial disclosure statement on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.

La. R.S. 42:1124.4 of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.

Recommendations:
Waive the late fee.
