Phillip David Ridder is requesting a waiver for the following reasons:
Mr. Ridder stated his neighborhood Post Office Mail Carrier (T. Neal) gave his NOD -FF and LFO notices to his grandson. Mr. Ridder stated he paid and gave his Tier 2.1 disclosure to Riverbend Accounting and Tax Service, Whitney Giluso, as she had helped him with his CF Reports. He contacted the BOE when he received a Demand Letter to inquiry why he was receiving a fine, and learned that his forms were never submitted to the BOE. Ms. Whitney Giluso also is requesting a waiver on behalf of Mr. Ridder. Ms. Giluso stated that Mr. Ridder would never purposely omit information or not file his disclosure. She contacted the BOE on behalf of Mr. Ridder when he received his NOD -FF and then she reached out to the Manufactured Housing Commission, but had to wait until they provided the information needed to complete his disclosure. Both Mr. Ridder and Ms. Giluso would like any consideration with this matter.
Staff Notes: Ms. Giluso did call the staff on 4/15/21, 3 days after receiving the NOD. Mr. Ridder did file a 2019 Tier 2 candidate PFD on 7/29/20 that disclosed the same information on his Tier 2.1 PFD. He also provided that his grandkids do not live with him but they stay with him everyday after school and believes they received both the NOD and the Late Fee Order.

La. R.S. 42:1124.2.1
requires financial disclosure for each member and designee of a board or commission which has the authority to expend, disburse or invest $10,000 or more of funds in a fiscal year on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
